Description


The Pharmacist is responsible for providing pharmacy consulting services to patients regarding the effective usage of medications, awareness with drug interactions and offering preventive healthcare services. Responsible for ensuring the proper dispensing, review, and verification of prescribed medications within regulatory guidelines, company policies and procedures. The Pharmacist will support the efficient workflow of the pharmacy and assist the pharmacist in charge with identifying ways to optimize pharmacy financials, inventory management, and the patient experience.


Essential Job Responsibilities


Patient Experience


• Engage patients by greeting them and offering assistance with products and services. Resolves patients issues in timely manner and answers questions to ensure a positive patient experience.

• Model and share patient service best practices with team members to deliver a distinctive and positive patient experience, including interpersonal habits (e.g. greeting, eye contact, etc.), proactively offering assistance, creating patient relationships that facilitate increased patient outcomes due to increased patient trust and understanding of patient medical situation and circumstances.

• Maintain client rights and responsibilities at all times.



Operations


• Counsel patients and answer their questions regarding usage of medicine, side effects, interactions, contraindications, over-the-counter products, and collaborates/refers to medical provider as needed to ensure medication is taken correctly, health needs are addressed, and patient is satisfied with service levels received.

• Perform pharmacist tasks including compounding, drug therapy reviews, verification, and medication management.

• Review, interpret, and accurately dispense prescribed medications.

• Ensure the pharmacy operates in accordance to regulations, Care Resource policies and standards.

• Assist pharmacist-in-charge with establishing procedures that promote efficient workflow in the pharmacy including assigning roles, coordinating activities and soliciting team member suggestions.

• Responsible for opening and closing of the pharmacy and shift change duties.

• Ensure the elements of Good Faith Dispensing are met in conjuction with state and federal controlled substance laws when filling prescriptions.

• Maintain current knowelge of pharmacy operating systems and technology to provide support to patients, as well as pharmacy team members.

• Propose and implement enhancements to pharmacy systems to further promote productivity.

• Ensure an accurate processing of insurance and 340B claims to resolve patient issues and prevent payment rejections or 340B compliance concerns.

• Follow up with insurance companies, as well as medical providers and participate in 3rd party audits.

• Follow up with medical providers to clarify prescribed medications, dosages, refills, interactions, and allergies to suggest alternative medications and answer medical provider questions.

• Perform clinical and wellness services such as immunizations, flu shots, diabetes awareness, and other preventive healthcare services.

• Assist and support the pharmacist-in-charge in analyzing performance data including pharmacy financial, patient service, and inventory management.

• Develop and maintain a good relationship with all staff and providers within Care Resource.

• Participate in company events.

• Maintain accurate and confidential records and documentation.

• Assist the pharmacist-in-charge with team member training and hiring.

• Reinforce the direction of the pharmacist-in-charge in team member performance by coaching, providing constructive feedback, monitoring patient service, and fostering team member development.

• Promote teamwork and motivate team members by establishing expectations, monitors, recognize progress, and foster a shared vision.



Training and Personal Development


• Maintain a current knowledge and skill related to pharmacy and healthcare by reading pharmacy related journals, company publications and communications.

• Remain current on all CE requirements required by state.



Communication


• Support the pharmacist-in-charge by communicating relevant health & wellness services or strategy information to team members and/or medical providers.



Quality Assurance/Compliance


• Complete all required training.

• Participate in regular case staffing to discuss procedures and quality assurance with the medical team.



Safety


• Ensure staff maintain agency guidelines relating to safety, outreach and confidentiality

• Ensure proper hand washing according to Centers for Disease Control and Prevention guidelines.

• Document patient medications correctly, makes sure each patient knows which medications to take when they're at home and encourage each patient to bring their up-to-date list of medicines every time they visit the doctor.

• Ensure each new client receive screenings for their risk of suicide.

• Understand and appropriately act upon assigned roles in Emergency Code System

• Understand and perform assigned roles in Agency Continuity of Operations Plan (COOP)

• Ensure outpost staff understand the safety requirements of the location in which they are posted.
